Approximate duration: first user impacted from ~8 April; surfaced and mitigated 20 April; affected users restored. Root-cause remediation on the provisioning script (idempotent upsert logic, scoped credentials, per-action audit logging) is in progress. A bulk membership-provisioning script in our identity layer incorrectly downgraded a small number of already-active members in one EU customer organisation back to a pending state, causing them to lose access to the application.
